Report Fraud, Waste, or Abuse

Custom Health is committed to ethical conduct, regulatory compliance, and transparency.

This reporting channel is intended for concerns related to fraud, waste, abuse, or other potential misconduct involving Custom Health operations, services, or partners. This form is not intended for customer service requests or prescription support.

Examples of reportable concerns may include, but are not limited to:

  • Suspected billing, reimbursement, or claims irregularities
  • Misuse, diversion, or improper handling of medications or devices
  • Falsification or improper alteration of records or documentation
  • Conflicts of interest or unethical business practices
  • Violations of healthcare laws, regulations, or internal policies

If you are unsure whether a situation qualifies as reportable, you may still submit a report for review.

Reports may be submitted anonymously, where permitted by law.

All submissions are treated confidentially and reviewed by appropriate compliance personnel. Retaliation against individuals who raise concerns in good faith is not tolerated.

After submission, reports are reviewed by compliance personnel and evaluated in accordance with applicable laws and policies. Submission of a report does not guarantee a specific outcome or response.
